Citibank was using Flexcube’s loan programme, which at the time was designed with a confusing interface. Three separate buttons labelled “override,” needed to be checked to pay only the interest. The default was to pay everything, unless overridden. The payment went through three levels of approval, and during this process, the staff simply overlooked two unchecked override buttons.
How did UAT fail?
• The user interface was designed in such a non-intuitive way, that it confused even experienced operators.
• In addition to the short comings of the interface, and importantly, no realistic test cases that replicated an “interest only payment” were included in UAT.
• The design relied on operators reading a manual which they did not.
Thus, the user acceptance testing conducted did not pick on this essential user experience issue, and the system went live without this issue being addressed,
leaving Citibank to incur a loss of $500 million at the time. In 2022, Citibank recovered its $500 million but its reputation was severely damaged.
